Registration 2011

Registration has been closed for the 2011 season

Registration prices for 2011 are:

  • $50.00 for the first player / cheerleader
  • 85.00 for 2 players / cheerleaders from the same immediate family living in the same household
  • $110.00 for three or more players / cheerleaders from the same immediate family living in the same household

When you register your child, Please be sure to bring a copy of your childs current REPORT CARD. In the next few weeks (we will be asking all Parents/Guardians to bring your childs ORIGINAL birth certificate.

The 49ers reserve the right to charge the full registration price if they determine that the players / cheerleaders are not members of the same immediate family living in the same household.

There will be an increase in the registration price after the first day of practice.

There are 2 ways to register:

  1. Download the forms below, fill them out completely, sign them, and mail them back to the Providence North End 49ers with a check or money order in the correct amount to:
